Our drow warlock Juniper died when another PC instigated a fight that didn't need to happen!! It ended up being pretty funny since our party hadn't known her for very long, so after her funeral another PC said, ".......i don't think she even liked us" and we all burst out laughing at how absurd it all was
had we played more games with her we probably would have been much more heartbroken though

•As a DM, last night, I had to kill one of my PCs for a multiclass thing they wanted. The character was a tiefling bard, and the player wanted to multiclass to shadow sorcerer. The player had befriended an NPC named Havoc, a tiefling ranger, and they were traveling along with him. The bard wanted to have revenge on some people that had klled his beloved one, and was collecting these sort of jade necklaces they owned. The thing was that, Havoc, had ulterior motives... and needed those necklaces for something else.
So last night my player got the last necklace, in a sort of way that made the party split. There was this orc living in the mountains, and ashamed of his past he was willing to talk with the PC. But the bard was merciless and killed the poor orc anyway. The party, not knowing this was going on inside the orc’s cabin, got pretty pissed cus “he was willing to talk, why did you kill him?!”. And so Havoc and Cas kinda ran away... whilst the rest of the party stayed behind to give a proper burial to the orc and apologize to the corpse.
When Havoc and Cas (the bard) were on their on, they talked about how they felt, and some other things of the past. Havoc kinda told Cas he was in love with him... but then backstabbed him with a paralyzing poison. As Cas remained on the floor, being unable to move, Havoc took the necklaces, putting them in a lead container. Had a little monologue, before delivering the final blows on “his friend”. Havoc walked away in Cas’es last minutes... when a bunch of ravens flew near the dying body of the bard. Cas stared deeply into one of the raven’s eyes... until everything he could see was darkness...
He found himself floating in this darkness, when a mask appeared before him (the Raven Queen) and that’s were I ended the session.
I told my player that she didn’t have to make a new character, and that she could multiclass to the type of sorcerer she wanted... but as it was 1:30 am we had to leave the session there hahaha

•As a player, I had a fellow player's character die due to a combination of me having moonbeam on the villain but then they moved, the player's character ran into the moonbeam for a bit, the villain had the BAD necrotic damage, and our dm accidentally lost track of said character's hp. We were all shocked when they said they were down, the dm included.
As a dm, i had a player and dmpc kidnapped and taken to vampires. They were under a special magic concoction that made them fall asleep (max 8 hrs). The party was already losing their minds because one was dating the dmpc and the other kidnapped character was a young halfling and everyone's figuratively adopted daughter/sister/niece. Long story short, the player and dmpc woke up to being chained to the wall and seeing they've been bitten two or three times. The player began pulling on their own chains to be free, taking damage from the struggle against their neck and wrists (the chains were magical and had a high DC). I had hoped with some owies they would get the hint to stop, but they kept trying in an emotional "we're going to get out of this" moment.... Only to die via the wounds. LOL. I was SO SHOCKED, i was laughing for 5 minutes straight. (This player is cursed with so many nat ones it's ridiculous. So many dice have gone to dice jail)
Now. This character is half Avariel... Meaning she has big beautiful wings on her back. So when she died, the vampires took her to a taxidermist (that they used suggestion on) to remove her wings. The rest of the party found the dmpc (who didn't tell them she died out of denial) and were able to get to her in time. The party went dead silence when it clicked that she'd died. When the bard/paladin spoke it was on the verge of crying. We had the player play an artificer so they wouldn't be sitting there bored while the others searched and they had made a little item that had the SMALLEST chance of bringing her back. They tried it and we got the successful roll but it had 4 potential side effects and she rolled the one that changes her eyes. It was rough for the character since her wings and her golden eyes were the only things she knew about her father.

•No, but another player’s character & their pet died. Their pet, a heavily obese solid white rat named “Gennish” was purchased at “The Fat Rat Pet Shop” by our Dwarven cleric, Sed E. Ment. (yes, his name was a rock pun!) After Sed gambled all of his money away & my character, Darragh, caused 2 very attention drawing incidents, including destroying that pet shop out of a magically induced panic attack, our party’s boss (a drow woman named Vallek) was pretty angry at us. After she angrily forced us to hold hands & walked us back to our “base,” an inn & we explained to her what all happened & she explained to us why she wants us working for her, we were all allowed to go our separate ways for the rest of the day. She also managed to steal back the money Sed had gambled away & (mistakenly) gave it back to him. Sed, with his rat stored in his beard, walked out of the inn & bumped into the guy who won Sed’s 100 gold & had the money stolen from by Vallek. The guy was mad & after escalating the situation a good bit, my character Darragh & Vallek went outside to see what the heck was going on. She cast “Lightning Whip” on Sed & restrained him, & the spell will also cause a bit of damage too. It only caused 1 point of damage, but the rat in his beard only had 1 hit point; taking the blow for his owner of 2-3 hours, which turned the rat into dust. It was pretty funny! Neither my character Darragh or Vallek knew it existed, so they were super confused when there was a puff of dust! Our party’s Bard, Atka, knew of the rat, as well as Sed so they were both somewhat comedically sad!
The player character died when (after a lot of stuff happened in several sessions) Sed willingly split from our party (& my character was magically separated in a completely different location) & started an uprising with the islands native barbarians. He led them to the strongest barbarian base out of all of the tribes. He challenged all of those barbarians to a fight, which he was outnumbered, the barbarians Sed was leading fled in fear since they were being treacherous, & he was promptly beaten to death like a little piñata. The player did this on purpose so he could play a different character, but after a few sessions of that character, he led the new character basically to death & left our group.

•Didn't happen to me, but to a party member, we were having a one shot campaign (which is pretty famous, so I'll refrain to spoil any detail!), and she was playing as a little aarakockra rogue, who basically got a bit too curious and died on the spot literally 15 minutes after the start of the first session! Tbh we were all in such disbelief that we all started laughing, the player took it pretty well too, she also already had another character ready, so it was all good :D
(Disclaimer: our DM did warn us that in this one shot it was really easy for a character to die, so we were ready for the eventuality!)

I had a warlock named Alicia, a Nobel who made a deal with a powerful fey woman who was slowly taking over Alicia’s body. Bear the end of the campaign a great power had destroyed a big city, killing everyone in it, we found an item of great power that was being used to destroy the world...the fey came to Alicia and said they could save everyone and bring back the city and everyone who died if she gave up her body. So she did. Now her body is wandering around with this fey woman behind the wheel and her own mind just...gone. It worked though...the city was back as if nothing had happened. It was such a bittersweet end to a campaign.

•My monk Morana has died 3 times now, each death in a different campaign. The first two deaths were TPKs, but the latest death was a rough one. Her group is currently under a curse the DM made called a spiders curse, and all their contacts are being attacked by a group of assassins. Once they figured out the next target, they rushed to try to save them, but were unable to, and Morana was killed mercilessly alongside the intended target. Her group didn’t even have time to mourn her death. Somehow though, despite being confirmed dead, she woke up fully alive in a morgue three days later (and not undead either). We’re still trying to figure out how exactly she is alive, but needless to say she is very unsettled by it.

•my rogue, Cuk, had his heart ripped out after he refused to take a demon’s shitty deal. The party had a short funeral for him where the Dragonborn monk played the bagpipes and his brother poured a bottle of wine into the grave. He got better tho :•)

•I was running a simplified Call of Cuthulu and a player was beaten to death by a giant forrest creature. This wasn't unexpected, as I did plan for at least one of them to die if they made poor decisions. I did make the character mysteriously come back to life a bit later. Adding to the mystery :D

•So i was playing a kenku warlock who was a total kleptomaniac bastard. We found a ring that would kill anyone who put it on instantly. He could not resist the shiny golden ring and immediately put it on.
