If you can log in, change the password and enable 2 factor authentication.
If you can't log in, there should be some way to recover the account? I'm not overly familiar with discord, but there should be a way for an account owner to get it back.
If you can get in, contact anyone you can telling them not to click anything you sent and THEY should change their passwords if they did.
