I haven't used Sai in a while, but I believe you take an existing brush, duplicate it, then adjust the settings of it. Back when I used deviantart, people would screenshot their pen settings for others to copy and adjust the same ways, so I think it's about pen setting adjustment vs making repeating pngs in photoshop or clipstudio for example. Hence why Sai doesn't have grasses brushes or anything either, it's more about adjusting the brush tips and flow
