Yesterday I saw how an artist I admire had some drawings reported from her private nsfw account. She has in her description several warnings about the type of art she does and that it is for +18 only.
It makes me sad that not even in a private account you can express yourself freely, because there are even people who will follow an account of this type if they don't even like the content, they will just report it :(

There was someone I recently followed that lost all of her following because of something happening to her account.
I believe some fear is healthy but fear keeping us back from what we want to do is something that we might regret in the future.
You could do something like cover up nsfw art and sell the full version on Patreon. There are going to be people that want to bring you down in life but you just need to keep going, so worst case scenario someone will report and maybe your account is banned, you just make another account. While there are people that for some reason would rather report than just go do something else, there will be tons of people that love seeing your art. I hope that puts you at some ease..!
